Complaint Form
Under the Statutes of the State of Ohio, the Board of Revision accepts original complaints filed between December and the end of the business day, March 31, only. Complaints filed before these dates will be returned to sender.
There are two options for completing the complaint form.
1. COMPLETE THE FORM ONLINE following these instructions:
a) If you know your parcel number, type it into the field below and the system will automatically populate the form with your property information. If you do not know you parcel number, click here to go to the Auditor's Property search page to locate this number.
b) Once your property information has been populated, continue completing the rest of the form online.
c) Once all information is complete, print the form, and make sure that your completed form is signed and notarized.

2. PRINT A BLANK COPY OF THE FORM
a) Download the complaint form and instructions.
b) Type or print your information legibly.
c)Once all information is complete, click 'view printable version' on the bottom of the form.
Make certain that your completed form is signed and notarized.
